SUBCONTRACTOR & EMPLOYEE FORMS
To avoid work and/or payment delays, please return each item in the following document checklist along with your signed Subcontract. All documents are required for the Subcontractor, and some projects may include Lower Tier Subcontractors.
Document Checklist -
- Request for Payment
- Change Order Request
- Waiver & Release Upon Progress Payment
- Certificate of Commercial Liability Insurance (spec)
- Contractor L&I Registration Certificate
- Proof of Workers Compensation w/Experience Factor.
- Completed W-9 Form or Employer Identification Number.

Sample of standard Accord certificate available from your insurance agent.
WSDOT 420-004 Subcontractor Certification for Federal-Aid Projects
Form is required for each Subcontractor or Lower Tier Subcontractor on all Federal-Aid projects.
WSDOT 421-012 Request to Sublet Work
Form is required when Prime Contractor is to sublet work to Subcontractor, Lower Tier Subcontractor or DBE on State or Federal projects.
WSDOT 350-071 Request for Approval of Material
Form is required for material or product/type approval with bid item and specification numbers, for State or Federal projects.

Sample form for documenting record of materials, State or Federal projects.
Form W-9 Taxpayer Identification Number
Must be provided in order to enter your company in Bianchi's accounting system. Required for both suppliers and subcontractors.
Form W-4 Employee's Withholding Allowance
Form is required by employer to withhold the correct federal income tax amount from your pay.
Form I-9 Employment Eligibility Verification
Form is required by employer to verify the identity and employment authorization of each new employee working in the United States.
Bianchi Safety & Health Policy
Required for all Bianchi employees. Includes safety meeting reporting sheet, and employee signature of awknowledgment.
